This agreement releases YOGA UNLEASHED RETREATS From any and all liability relating to injuries that may occur during any and all activities, related or pertaining to YOGA UNLEASHED RETREATS, PERU EXPERIENCE, Held at the Samadhi Center, Sacred Valley, Peru, Machu Picchu, Humantay lake, Sirenacocha Lake Day Hike, Maras, Moray & Salineras Excursions.
Upon signing this agreement I:WE agree to hold YOGA UNLEASHED RETREATS Entirely free from and liability, including financial responsibility for injuries incurred, regardless of wether injuries are caused by my own negligence.
I also acknowledge the risks involved in all activities taking place at the SAMADHI CENTER, SACRED VALLEY PERU, AND ALL EXCURSIONS.
These include but are not limited to sprains, trips, falls, lost or stolen items, car accident, death.
I:WE am participating voluntarily, and that all risks have been made clear to me. Additionally, I:WE do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. By signing below I:WE waive all liability against YOGA UNLEASHED RETREATS for any reason.
In return, I will receive participation any and all activities at Samadhi Center, Sacred Valley Peru, All Excursions, including Machu Picchu and Lake Humantay Sirenacocha Lake Day Hike, Maras, Moray & Salineras Excursions.
I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed and reserve the right to withdraw from the above experiences at anytime of my own free will and choice, with no expectation of refunding of placed funds paid in advance or on sight for said excursions.
I am of legal age and affixed my signature hereto.
