Waiver of Liability for Holistic Detoxfication Program
I hereby acknowledge that I voluntarily agree to attend and/ or participate in a paid Natural Detoxification program with Jason Kerr from Holistic Panama ..This program will comprise 4 Months of work conducted long distance and paid at 4$K .... First Initial payment was 2/23/19 for 750$ .. A second payment will be made by March 3rd for 300$ ..
Our work going forward will comprise of regular scheduled healing session , naturopathic and detox guidance along with Subtle Energy & Shamanic Healing to the completion of the program and fulfilling of client & practitioner goals ....
- I recognize and agree that coaching and educational services are intended to support holistic healing and soul healing and at times may require emotional, physical, spiritual, and mental efforts, exertion, and behavioral experimentation on my part, which may cause physical, mental or emotional injury, and I fully acknowledge and agree to take full responsibility for any risks involved. I understand and agree that I am fully responsible for my well-being during my coaching and educational services sessions or my attendance or participation in any classes, workshops, or other programs, and subsequently, including my choices and decisions.
- I agree to take full accountability for the outcome of my participation in this experimental and educational activities with Lauren Barta, Xolar Vibronics, Consciousness Is Healing llc. And that I am requested and responsible to immediately communicate to my wholesome life coach if ever something comes up in which I feel that I am having challenges or feeling uncomfortable in the process in any way as clients having a good professional and positive outcome is always of the highest intent and regard.
- I understand and agree that coaching is, at present, an unregulated industry and that while my Holistic Coach is certified through the Xolar Vibronics Training Center, this is a field that is not licensed by any state.
- I hereby release, waive, acquit and forever discharge jason kerr / Holistic Panama , Xolar Vibronics any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by my wholesome life coach as a result of the educational information and advice given by my wholesome life coach or otherwise resulting from the coaching relationship contemplated by this agreement. This agreement shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
- I further declare and represent that no promise, inducement or agreement not expressed in this agreement has been made to me to sign this agreement.
- I have the legal ability to give my informed consent, and hereby give such consent to my holistic life coach to educate and assist me towards achieving my goals.
- I, the client, understand and agree that by filling out my contact information below and signing this form, that I have reviewed, understand and agree to each of the statements and the terms and conditions stated above. I am requesting coaching , natural detoxification and educational services from Jason Kerr /Holistic Panama and Xolar Vibronics,..
