I, as the name listed below, attest that: I am at least the age of majority in my state of residence.
I am submitting this Release with the understanding that it will be relied upon to determine whether I am entitled to receive the prize for which I am the apparent winner in the MetroPCS “UFC 220 Watch Party” sweepstakes (the “Sweepstakes”) sponsored by MetroPCS Massachusetts, LLC ("Sponsor"). I understand that if I do not fully complete, sign and return this Release by the redemption deadline set by Sponsor I will forfeit any prize that I may otherwise have received.
I represent that I have read, understand and complied with the Official Rules of this Sweepstakes, and that I have perpetrated no fraud or deception in entering or in claiming my prize. I further represent that I meet all eligibility criteria set forth in the Official Rules, including, without limitation, the requirement to be a legal resident of one of the areas listed in the Official Rules. I further represent that neither I nor any member of my immediate family is an employee of the Sponsor, Sponsor’s authorized dealers, their respective parents, affiliates, subsidiaries, prize providers or advertising or promotions agencies, or of any other entity specifically excluded in the Official Rules. I agree to return to the Sponsor any prize which may be awarded to me if any statement made by me in this Release is false.
I understand that the prize consists only of two (2) tickets to the UFC 220 event on January 20, 2018 at the TD Garden, Boston, MA (the “Event”), and that no other expenses, including, without limitation, food, beverages, gratuities, taxes, parking, hotel accommodations, ground transportation, souvenirs, retail purchases, or incidentals are included in the prize, all as set forth in the Official Rules. If I am unable to use the prize or any portion thereof for any reason, I understand that I will forfeit the prize and no other prize will be given to me. I further understand that if the Event or any pre-Event or post-Event activity does not take place for any reason, Sponsor will not provide me with a substitute prize or any other compensation. I understand that Sponsor makes no representations or warranties of any kind regarding my prize.
I agree to indemnify and hold the Sponsor, Sponsor’s authorized dealers, and Zuffa, LLC, together with their respective parents, affiliates, subsidiaries, prize providers or advertising or promotions agencies and their respective directors, officers, employees, licensees and assigns harmless against, and release them from, all and any claims and injuries which may arise from or be based upon my winning, acceptance of or use of the prize I receive in connection with the Sweepstakes, including, without limitation, all and any liability for personal injury, property damage, death, illness, or other injury or accident, whether to me or to any person that I include in any prize-related activity, or other liability of any kind.
I acknowledge and agree that I am solely responsible for any taxes on the value of the prize, and that if legally required Sponsor will file a Form 1099 with the Internal Revenue Service. I agree that if so requested by Sponsor I must provide Sponsor with my social security number as a condition of receiving the prize.
I do hereby give Sponsor, prize providers for the Sweepstakes, and their respective parents, affiliates, agents and assignees, full permission and authority to use, publish and display my name, voice and photograph or other likeness for advertising or other related purposes in any media without additional compensation, unless prohibited by law.
