The Adhesive used for eyelash extensions is a medical grade and will only to be applied to lashes during treatment, not the skin. It is very unlikely that clients will experience an allergic reaction, however it is recommended that each client be patch tested at least 48 hours prior to treatment.
Please complete the following to confirm that you give your permission for a patch test to be carried out by having a small amount of adhesive to you skin (behind the ear). If there is any sign of redness, itching, swelling or blistering, please return to have any traces of tested product removed and do not proceed with the treatment.
This agreement releases The Lash Lab from all liability relating to reactions that may occur from a patch test for semi permanent lash extensions. By signing this agreement, I agree to hold The Lash Lab entirely free from any liability, including financial responsibility for reactions incurred.
I also acknowledge the risks involved in patch testing. These include but are not limited to Redness, itching, swelling or blistering. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing reactions while engaging in this activity.
By signing below I forfeit all right to bring a suit against The Lash Lab for any reason.
