Thank you for your interest in a class or event at Studio15!

Please review and sign the following Participant Consent and Waiver of Liability – Indemnity Agreement prior to you/your child’s participation in activities at Studio15. This is a one time wavier and will be kept on file.

I, the undersigned parent or legal guardian of the participant, or the participant myself, understand that I am solely responsible for the delivery and the pickup of the participant at the Studio15 facility. I agree that Studio15 assumes no responsibility for the supervision of the participant at times other than during the class sessions. I hereby waive and release any right I may have or acquire to make a claim against or attach the property of Studio15 or any of its members, employees, or agents for monetary damages caused by injury to the participant or damage to the property of the participant or myself arising from participating in the activities and use of the facilities and property of Studio15.

I have carefully read this agreement and fully understand its contents. I am aware the agreement
includes a waiver of liability and indemnity agreement.
