International Sports Connections Inc considers safety to be its utmost concern. The customer agrees to assume full responsibility for any risks, injuries, damages, known or unknown which might occur as a result of renting.

The customer releases International Sports Connections Inc "SoSUP" from any Liability involving his/her choices and decisions during and after the rental.

1. I hereby agree to indemnify and hold harmless International Sports Connections Inc, and staff, and the State of Florida and all of the State’s representatives, and the Department of Natural Resources from any and all liability for any harm, injury, death, or damages (physical, mental, or monetary), regardless of fault or negligence, which may befall me while participating in this program or while using International Sports Connections Inc facilities. This release shall be binding upon my family, heirs, and administrators.

2. I hereby agree to heed at all times the advice and instructions of International Sports Connections Inc staff and to practice common sense water safety procedures.

3. To the extent provided by State Law, I also freely and voluntarily waive all claims for damages or loss to my person or property, or the person or property of third parties, that may be caused by an act, or failure to act, of the Florida or its agents and employees, and release the Florida and the State of Florida from any and all claims.

I certify that the above INFORMATION is correct to the best of my knowledge:

This agreement releases {Organization} from all liability relating to injuries that may occur {during activity, on location, etc.}. By signing this agreement, I agree to hold {Organization} entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
