I authorize AZE Wireless technician(s) to perform work on my device. I understand that AZE Wireless technicians have been trained to perform a hardware and software work, but AZE Wireless is not an authorized service dealer. Further, I agree to release, indemnify, and hold harmless AZE Wireless from liability for any claims or damages of any kind or description that may arise from any repair work performed on my device unless it is caused by severe negligence of AZE Wireless or its agent. I understand that AZE Wireless is not responsible for any data loss, which may occur as a result of work done on my device.
I certify that I am the expressed owner of the device described in this document. In the event that I am found not to be the true expressed owner of this device, I assume all liability for any claim made as the result of the technical support rendered by AZE Wireless on this device, including those claims which assert negligence on the part of AZE Wireless.
I understand that technical support rendered by AZE Wireless may void manufacturer warranties for this device. AZE Wireless does not assume any liability or warranty in the event manufacturer warranties are voided.
I understand that if I bring a non working device to AZE Wireless and technician can not check the functionality of device before repair, AZE Wireless is not responsible for any damage on this non working device after repair is done.
Repairs can not be exchanged or refunded. Only the part that was replaced is under warranty within 30 (thirty) days if there no physical or water damage and in original condition. AZE Wireless does not take any kind of responsibility for any other parts of the phone that were not replaced by our technicians or the software issues on the device.
