Participant’s Release In acceptance of this application, I intend to be legally bound and do hereby for myself, my heirs and assignees, waive, release and forever discharge any and all rights and claims for damages, which may hereafter occur to me, against the Carleton Place Canoe Club, Canoe Kayak Canada and any Division or affiliated Canoe Club, any coach or officer of any such organization in connection with paddling, weight training, swimming, fitness or other programs, or which may arise out of any training program or activity, travelling competitive event or related activities.
I understand that participation in sports activities creates an inherent high degree of risk of injury. I hereby agree to assume and be responsible for all risks and hazards incidental to such participation. If I should sustain any injury while participating in this program, I hereby give permission for me to receive emergency health care, including transportation to the nearest emergency health care unit, as may be deemed appropriate by the coaches or supervisors of the Carleton Place Canoe Club or any medical health care provider. I agree to be responsible for all costs, which may arise from such emergency treatment. It is understood that the participant’s signature below indicates that I can swim adequately, and that the Carleton Place Canoe Club will not be liable for property loss or any injury arising from participation in any program, or use of any equipment of the Carleton Place Canoe Club.
CPCC Code of Conduct and Disciplinary Procedures The Carleton Place Canoe Club is a place where all members must feel welcome, valued, and safe. The club has a code of conduct for all members. This includes, athletes, campers, parents, volunteers, and staff members. It is required that at all times, participants and parents must behave responsibly and appropriately in a respectful and positive manor that creates a positive canoe club environment. Everyone is expected to abide by this code of conduct, and any violations will be subject to review and, where necessary, further disciplinary action.
Code of Conduct 1. As a CPCC member participant; 1.1) I will not commit physical, sexual, or verbal harassment in any way, shape or form 1.2) I will not vandalise canoe club property or facilities. I will pay the cost of damage repairs or replacement incurred by my act(s) of vandalism 1.3) I will be respectful to all coaches, staff, board members, peers, and volunteers within the canoe club community 1.4) I will adhere to CPCC’s equipment usage guidelines, taking great care while using and/or storing CPCC equipment 1.5) I will ensure that my behaviour is respectful, positive, and sportsmanlike while participating in any canoe club activity, competition, or training event
2. As a CPCC parent of a CPCC member participant; 2.1) I will not commit physical, sexual, or verbal harassment in any way, shape or form 2.2) I will not vandalise canoe club property or facilities. I will pay the cost of damage repairs or replacement incurred by my act(s) of vandalism 2.3) I will be respectful to all athletes, coaches, staff, board members, peers, and volunteers within the canoe club community. We have a zero tolerance policy on this matter. 2.4) I will not disrupt the daily training environment in any way. This includes, but is not limited to: 2.4)1. Communicating with athletes during program time (emergencies excluded) 2.5) I will not openly criticize any volunteer official at competition events 2.6) I will adhere to the CPCC Parent Communications guidelines as outlined in the club handbook for paddlers, parents, and volunteers
3. Violation of the CPCC Code of Conduct will be documented using a CPCC incident report form. The following disciplinary procedure will be implemented: 3.1) Verbal warning 3.1)1. Directly with violator 3.1)2. Directly with parent(s)/guardian where appropriate 3.2) Written warning (to parent/guardian if under the age of 18) 3.3) Phone call home 3.4) In person meeting with parent(s)/Guardian 3.5) Suspension from program participation (length of suspension is to be determined by the CPCC disciplinary committee 3.6) Denial of access to club premises 3.7) Removal from CPCC programming without refund
Declaration: I have read and understand the Carleton Place Canoe Club Code of Conduct and I agree to adhere to it during my time in/at CPCC programs and events, both on-site and off-site. This agreement is applicable at all canoe club related programs, events, activities, and competitions. It is applicable to on-site as well as off-site events while representing the CPCC. I the undersigned hereby agree to abide by all of the rules of the Carleton Place Canoe Club.
