By signing, I confirm that I have received my MBKF wristband/stamp and understand if I lose my wristband, I will have to pay the full price of the pass in order to receive a new wristband.
Release of Liability:
I, (the customer), have registered for the Miami Beach Kizomba Festival offered by Cedgy Promotions, Inc. I recognize that the event may involve strenuous physical activity including but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this event. I acknowledge that my enrollment and subsequent participation is purely voluntary and in no way mandated by Cedgy Promotions, Inc. In consideration of my participation in this program, I (the customer), hereby release Cedgy Promotions, Inc. and its agents from any claims, demands, and causes of action as a result of my voluntary participation and enrollment.
I fully understand that I may injure myself as a result of my enrollment and subsequent participation in this event and I, (the customer), hereby release Cedgy Promotions, Inc. and its agents from any liability now or in the future for conditions that I may obtain. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knee, injuries to back, injuries to foot, or any other illness or soreness that I may incur, including death.
Media Release:
I, (the customer), hereby authorize Cedgy Promotions to photograph me, take motion pictures of me, take video footage of me, and/or make electronic sound recordings of me (herein referred to as photographic or electronic reproductions).
I authorize the use of any such photographic or electronic reproductions of me for any purpose, including, but not limited to educational and other public media as may be deemed appropriate by Cedgy Promotions (I understand that I may be identifiable from such photographic or electronic reproduction).
Behavior:
Participants are expected to behave responsibly and to treat each other – and treat the community – with respect, kindness, and compassion. MBKF reserves the right, without refund, to revoke the passes of Participants whose conduct is deemed inappropriate, disorderly or offensive by MBKF, affiliated third parties or the local authorities.
