MHS Cheer Waiver of Liability and Survey
This agreement releases Bronco Cheer Club and any of its members and officers from all liability relating to injuries that may occur during Cheer events. By signing this agreement, I agree to hold Bronco Cheer Club, Oklahoma Family Counseling Services LLC., All America Bank any of its officers or members entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in Cheer events including but not limited to, Fireworks,Senior Sneak, camps or any other fundraising events, transportation provided by athletes, cheer families, coaches and or sponsors. I swear that I am participating voluntarily. By signing below I forfeit all right to bring a suit against Bronco Cheer Club and its’ officers or members for any reason. Participation in the event and any activities within is completely voluntary and I can decline to participate at any time. I will also make every effort to obey safety precautions. I will ask for clarification when needed.
