This contract outlines an agreement and all contractual obligations made on February 1, 2012, between Skin Occasions makeup artists and Erin Cooper, of 6127 Rustic Creek Lane, Kingwood, 77345, here referred to as bride.
RELATIONSHIP OF PARTIES
Skin Occasions is not to be considered an agent or employee of bride for any purpose. The parties intend that an independent contactor relationship will be created by this contract. Bride is interested only in the results to be achieved; the conduct and control of the work will lie solely with Skin Occasions.

WEDDING INFORMATION
Number in bridal party getting makeup done:
BRIDAL QUESTIONARRE
PAYMENT
If service is not booked in our makeup salon and we need to travel to your location a travel and set up fee will be accessed. A minimum of 4 bridal party members (including bride) is required for travel.
Travel/Set up fee: $50
Bride: $100
Additional party members: $65 each @ ___ people
Total fees: $410 for 4 people
Full payment is required in advance to book. Payment is due on: April 15, 2012
Services will be completed in the time allotted. Any additional time desired will be charged at $100 per hour.
Time allocated for services and set up: 4 ½ hours
Start time: 10:00 am
End Time: 2:30 pm
If for any reason, the date is cancelled without a 3 week notice, the payment is forfeited.
ENTIRE AGREEMENT
This agreement represents all the terms and conditions agreed upon by the parties. No other understanding, oral or otherwise, shall bind any of the parties.
