1. In order to enjoy a relaxing spa experience and as a courtesy to other clients and
tenants of the spa, the use of cell phones is prohibited in the spa hallway, waiting area or
treatment rooms. Please turn your phone to silent before entering the spa hallway. If
you would like to use your phone we will be glad to escort you to a private area.
2. Unfortunately, infants and children younger than 14 are not allowed within the
spa waiting area or treatment rooms. We ask that you provide arrangements for your
children as a courtesy to others who are enjoying their spa services or to other tenants in
the spa.
3. Clients should expect to arrive 5-10 minutes prior to their appointment start time.
We start all appointments on time to ensure proper service completion and happy
satisfied clients.
4. If you expect to be more than 5 minutes late, we ask that you call and advise us
accordingly. This way we can determine if the remaining time scheduled is enough to
allow the service to be completed to both the satisfaction of the client and that of the
servicing esthetician. If not, we will offer you a rescheduled start time.
Appointment Confirmation/Cancellation Policy
1. It is the practice of Skin Occasions to provide ‘two days before’ reminders for all
appointments. We ask that you please respond your confirmation via text or phone call.
Please note that failure to respond to our text or phone call reminders may result in your
appointment being cancelled by us. This is done to allow other clients to have access to
the available appointment time.

3.Same day cancellation or “No Show” appointments will be subject to a service and/or rescheduling fee being 50% of service.
4. Multiple cancellations or “No Shows” will result in pre-payment for the service
requested and would be non-refundable.
I authorize MARGIE ORTIZ-CHEATUM, and Rebecca Medina (Lash Stylist) and Skin Occasions to provide and apply semi-permanent eyelash extensions. In order to minimize risk of eye injury, I understand that it is my responsibility to lie completely still for the entire eyelash extension procedure or until otherwise directed by the Lash Stylist. I have been fully informed regarding the methods and procedures used in the application of semi-permanent eyelash extensions. All risks and potential complications, including but not limited to redness, irritation, and allergic reactions to either the adhesive and or eye pads, have been fully disclosed to me. I certify that I understand these risks and potential complications, and that I knowingly and voluntarily consent to the application of semi-permanent eyelash extensions.
If at any time I feel uncomfortable during the lash procedure, I agree to immediately notify the Lash Stylist. The Lash Stylist will seek to remedy the problem, including ending the session if it is deemed prudent by the Lash Stylist. I acknowledge that no guarantees or promises regarding the appearance or longevity of the lash extensions have been made. I herby certify that I have disclosed all conditions regarding my health history, medications and past reactions to products, treatments and medications in connection with the Confidential Client Profile and Consultation Form.
I agree to use only recommended products on my eyelash extensions and fully understand that careful maintenance is also required.
During the Initial 24-48 Hours, I agree to:
Avoid getting the lashes wet for the first twenty-four hours following my lash extension application.
Avoid steam from showers, saunas and swimming pools, and avoid suntan beds for forty-eight hours following the application.
I certify that I understand, and agree to comply with the following instructions with respect to the care of my lash extensions: (check)
Avoid oil based skincare, mascara and cleansing products. ONLY water-based oil-free products are approved. Oil based products, including oil-based eyeliner, will affect the adhesion and cause the lashes to fall out prematurely. Only approved lash extension mascara is permissible. Oil-based and or waterproof mascara will interfere with the adhesive bond with disastrous results.
Keep your fingers off your lashes. Every time you touch them you are transferring natural oil and GERMS.
Brush your lashes with a mascara wand to groom your lashes, ideally when your lashes are wet or when you step out of the shower. The lashes will be softer and more supple.
Use ONLY heated eyelash curlers. The mechanical curlers will break the bond. When using your heated eyelash curler, brush & smooth your lashes all in the same direction. Use a clean mascara wand to support your lashes and curl lashes in an upward motion.
Use ONLY powdered, liquid or gel eyeliner. When cleaning your lashes, gently rub them between your forefinger and thumb using ONLY approved make-up or mascara remover/ cleaner
Inform all your services providers regarding your semi-permanent lash extensions when going for facial work. Caution is recommended when having services such as lasers, chemical peels, hydroxyl treatments, or any other acidic substances applied to your face.
I understand that the natural lash cycle is sixty to ninety days. When one lash falls out, there is another lash growing in. I, therefore, understand that I will need regular refills (every 2-3 weeks) and touch-up appointments to keep my lashes looking beautiful. I understand that if I come after the (3week) period the Lash Stylist will need to have extra time to replenish the lash, therefore their might be an additional charge. I understand that variables, including the natural lash cycle and customer care, will influence the longevity of eyelash extensions.
Lash Allergy vs Lash Reaction
It is important to know the signs of an allergic reaction and to be able to differentiate between an allergy and irritation. While some of the warning signs between an irritation and an allergy may be similar, there are a few key differences between them.
Allergic Reactions:
Allergic reactions to lash extensions are rare and can show up in different forms. The most common reactions are: swollen eyes/eyelids, redness of the cornea, itchiness and/or discomfort around the eye area. These symptoms are likely to show up within the first 48 hours from the lash application.
Irritations:
While irritations can have similar signs to allergies, they are often less severe and subside after the adhesive is cured. This occurs typically within the first 24 hours post application. Often even just hours after. Irritations that can cause redness and itching. These irritations are likely to subside quickly and become more mild as time goes on.
If you are having too much discomfort, it would be best to remove the lash extensions as soon as possible. Skin Occasions does charge a $50 removal fee for lash removal.
If you start to experience a severe reaction, or have any health concerns that may make them more susceptible to reactions, it is recommended that they consult with a healthcare professional.
BY SIGNING THIS CUSTOMER WAIVER AND RELEASE AGREEMENT, I THE CLIENT NAMED BELOW CERTIFY THAT I KNOWINGLY AND VOLUNTARILY RELEASE MARGIE ORTIZ-CHEATUM, (LASH STYLIST) AND SKIN OCCASIONS AND ITS DIRECTORS, OFFICERS, OWNERS, EMPLOYEES, AGENTS AND REPRESENTATIVES FROM ANY AND ALL CLAIMS FOR DAMAGES FOR PERSONAL INJURY ARISING FROM THE APPLICATION AND USE OF SEMI-PERMANT EYELASH EXTENSIONS, INCLUDING ANY DAMAGES RELATING TO KNOW OR UNKNOWN COMPLICATIONS WHICH MAY ARISE DURING OR FOLLOWING THE APPLICATION PROCESS INCLUDING BUT NOT LIMITED TO CLAIMS FOR NEGLIGENCE. I FURTHER RELEASE AND HOLD HARMLESS MARGIE ORTIZ-CHEATUM (LASH STYLIST) AND SKIN OCCASIONS FROM ANY CLAIMS RELATING TO PREEXISING CONDITIONS I HAVE NOT REVEALED OR CHANGES TO THOSE CONDITIONS SUBSEQUENT TO THE PROCEDURE.
I AGREE THAT ANY DISPUTES BETWEEN MARGIE ORTIZ-CHEATUM (LASH STYLIST) AND/OR SKIN OCCASIONS, SHALL BE SETTLED BY ARBITRATION ADMINISTERED BY THE AMERICAN ARBITRATION ASSOCIATION IN ACCORDANCE WITH ITS COMMERCIAL ARBITRATION RULES, AND JUDGMENT ON THE AWARD RENDERED BY THE ARBITRATOR(S) MAY BE ENTERED IN ANY COURT HAVING JURISDICTION THEREOF. THE PREVAILING PARTY SHALL BE ENTITLED TO REASONABLE ATTORNEY FEES AND
ARBITRATION COSTS INCURRED IN CONNECTION WITH ENFORCING THIS AGREEMENT.
I, as the name listed below (“Client”) certify that I have read and fully understand this Customer Waiver & Release Agreement. I further certify that I have completed the Client Confidential Profile accurately and completely to the best of my knowledge, and that I understand the potential complications and risks described herein. I hereby authorize MARGIE ORTIZ-CHEATUM (Lash Stylist) and Skin Occasions to provide and apply semi-permanent eyelash extensions to my own natural lashes, in accordance with the terms and conditions set forth in this Customer Waiver and Release Agreement.
