Waiver of Liability and Hold Harmless Agreement
In consideration for receiving permission to participate, I hereby release, waive, discharge and covenant not to sue The Shreveport Aquarium and Planet Aqua Group its officers, servants, agents, and employees (hereinafter referred to as “Releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees, or otherwise while participating Saturday, February 18 and/or Saturday, February 25 in activities in conjunction with the Guinness World Record Mexican Wave Line Attempt.
I also acknowledge the risks involved in the Guinness World Record Mexican Wave Line Attempt. These include but are not limited to any activities or operations associated with the operation of the Shreveport Aquarium activity including use of equipment, materials, fixtures, food, and furniture. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.
By signing below I take full responsibility and forfeit all right to bring a suit against the Releasees for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
