Select "GelCity - Waiver" if you are over the age of 18 and are not needing to sign on behalf of an accompanying minor.

Select "GelCity - Waiver (signing on behalf of a minor/minors) if you are playing with or supervising a minor or group of minors. 

You will be asked to provide your full name, contact, emergency contact information and your signature. Please provide the information accurately only after thoroughly reading the agreement as this waiver is subject to change. 

If you are unsure if you have already signed a waiver or worried your waiver may be expired, please select either waiver and search your contact information. You may 'renew' if your information appears, if not please fill out the appropriate waiver.
